A Note on How We're Regulated

We believe in transparency, so here it is plainly: as a cash-based practice, we do not bill insurance companies electronically, which means the federal privacy law known as HIPAA generally does not apply to our practice. That does not lower our standards — it means our standards are our own, and we set them high. Your information is protected by Utah law, by the professional confidentiality obligations of Dr. Danaya's physical therapy license, and by the commitments in this policy. We treat everything you share with us the way clinical information should be treated: as confidential.

Health & Personal Information

In the course of providing physical therapy services, we may collect the following types of information:

Personal Identifiers: Name, date of birth, address, phone number, email address, and emergency contact information
Medical Information: Health history, diagnoses, treatment plans, progress notes, and clinical assessments
Billing Information: Payment method details, insurance information you provide for superbills, and transaction records

When you visit our website, we automatically collect certain usage information, including:

Treatment: To provide, coordinate, and manage your pelvic floor physical therapy care, including consultations, evaluations, and follow-up sessions
Payment: To process payments, generate invoices, and prepare superbills at your request so you can seek reimbursement from your insurance company
Communication: To respond to your inquiries, schedule appointments, and send appointment-related messages. We will not send you marketing text messages without your express written consent
Practice Operations: To maintain records, improve our services, and meet our legal and professional obligations

We never sell your information. We never use your health information for marketing. If we ever want to share your story or feedback publicly — for example, as a testimonial — we will ask for your express permission first, and you are always free to say no.

We share your health information with others only when you ask us to or agree to it — for example, providing a superbill for you to submit to your insurer, or coordinating your care with another healthcare provider you have approved.

As Required by Law

Like every healthcare provider, we may be required to disclose information in limited circumstances, including:

Mandatory reporting required of licensed healthcare providers under Utah law

Access to Your Records

You have the right to inspect and receive a copy of your treatment records. We will respond to your request within 30 days. We may charge a reasonable fee for copying and mailing.

Corrections

If you believe something in your record is inaccurate or incomplete, you may request a correction. If we decline the request, we will explain why in writing.

How We Contact You

You may ask us to communicate with you in a specific way or at a specific number or address — for example, email only, or only at a particular phone number — and we will honor reasonable requests.

Opting Out

You may opt out of any non-essential communications at any time. You may also ask us to delete contact information we hold about you that is not part of your clinical record. (Clinical records themselves must be retained as described under Data Retention below.)

Vendor Selection: We choose service providers with privacy and security in mind, and we limit what each one can see to what its job requires

Website Privacy, Cookies & Analytics

Our website uses cookies and similar technologies to understand how visitors use the site and to measure our advertising. Specifically, we use Google Analytics and Google Tag Manager (site usage), Microsoft Clarity (anonymized usage insights), and conversion measurement for ads we run. These tools receive information about pages visited and site interactions — they do not receive the content of the messages you send us.

You can control cookies through your browser settings, and you can opt out of Google Analytics with Google's browser add-on. Browsing this website with cookies disabled will not affect your ability to contact us or receive care.

Third-Party Services

We retain clinical records for at least seven years from the date of your last treatment — and longer for minors, as Utah law requires — consistent with professional record-keeping standards for physical therapists. After the retention period expires, records are securely destroyed.

In the unlikely event that your information is exposed through a security breach, we will notify you promptly in writing, consistent with Utah's data breach notification law. The notification will describe what happened, the types of information involved, the steps we are taking in response, and what you can do to protect yourself.

We may update this Privacy Policy from time to time. When we make changes, we will update the "Last Updated" date at the top of this page. We encourage you to review this policy periodically to stay informed about how we are protecting your information. Continued use of our services after changes are posted constitutes your acceptance of the updated policy.
