Welcome to Radiant Pelvic Health & Wellness. These Terms of Service ("Terms") govern your use of our physical therapy services, website, and any related communications. By scheduling an appointment, receiving treatment, or using our website, you agree to be bound by these Terms. Please read them carefully before using our services.
1. Services Provided
Radiant Pelvic Health & Wellness is a cash-based pelvic floor physical therapy practice owned and operated by Dr. Danaya Kauwe, PT, DPT, PRPC. We provide specialized pelvic floor physical therapy services, including but not limited to:
- Initial evaluations and comprehensive pelvic floor assessments
- Ongoing treatment sessions and individualized care plans
- Manual therapy techniques, including internal and external pelvic floor treatment
- Therapeutic exercise prescription and movement training
- Dry needling for pelvic floor and related musculoskeletal conditions
- Patient education on pelvic health, self-management strategies, and wellness
2. Eligibility & Patient Responsibilities
2.1 Age Requirement
Our services are intended for individuals 18 years of age and older. Minors may receive treatment with the written consent of a parent or legal guardian, who must be present during all appointments. The parent or guardian assumes full responsibility for the minor's care and payment obligations.
2.2 Patient Responsibilities
As a patient, you are an active participant in your care. To ensure the best possible outcomes and a safe treatment environment, you agree to:
- Provide accurate and complete information about your health history, current symptoms, medications, and any changes in your condition
- Follow your individualized treatment plan and home exercise program as prescribed
- Communicate openly with your therapist about your comfort level, pain, concerns, or questions during treatment
- Arrive prepared for your appointment with appropriate clothing and a clean, safe treatment space for in-home visits
- Maintain a safe and respectful environment for your therapist during in-home sessions, including securing pets and ensuring a clear treatment area
- Pay for services at the time of your appointment unless other arrangements have been made in advance
3. Scheduling & Appointments
3.1 Scheduling
Appointments can be scheduled by contacting us directly via phone, email, or through our website. We will do our best to accommodate your preferred date and time. Appointment availability is subject to the therapist's schedule and geographic service area.
3.2 Cancellation Policy
We understand that life happens and schedules change. However, because our practice reserves dedicated time for each patient, we require 24 hours' notice for any cancellation or rescheduling. Cancellations or rescheduling requests made with less than 24 hours' notice will be charged 50% of the session fee. Missed appointments without any prior notice (no-shows) will be charged the full session fee. We reserve the right to waive this policy at our discretion for emergencies or extenuating circumstances.
3.3 Late Arrivals
Because our sessions are conducted in your home, please ensure you are ready at the scheduled appointment time. If you are not available when your therapist arrives, the session time will not be extended. If your therapist is unable to begin the session within 15 minutes of the scheduled start time, the appointment may be considered a no-show and the full session fee may apply.
4. Payment Terms
4.1 Cash-Based Practice
Radiant Pelvic Health & Wellness is a cash-based (out-of-network) practice. We do not bill insurance companies directly. Payment in full is due at the time of each appointment. By scheduling an appointment, you agree to pay for services rendered regardless of any insurance reimbursement you may or may not receive.
4.2 Accepted Payment Methods
- Credit and debit cards (Visa, Mastercard, American Express, Discover)
- Health Savings Account (HSA) cards
- Flexible Spending Account (FSA) cards
- Cash or check
4.3 Insurance Reimbursement
While we do not bill insurance directly, we provide superbills (detailed receipts with medical codes) upon request so that you may submit claims to your insurance company for potential out-of-network reimbursement. Reimbursement amounts vary by plan and are not guaranteed. It is your responsibility to verify your out-of-network benefits with your insurance provider prior to beginning treatment.
4.4 Pricing
Current pricing for our services is listed on our website. Prices are subject to change with a minimum of 30 days' written notice. Any price changes will not affect previously scheduled and confirmed appointments. Package pricing, if offered, must be used within the timeframe specified at the time of purchase.
5. Scope of Practice & Treatment
5.1 Physical Therapy Services
All services provided by Radiant Pelvic Health & Wellness are within the scope of physical therapy practice as defined by the State of Utah. Our services are not a substitute for medical diagnosis, treatment, or advice from a physician or other healthcare provider. We do not prescribe medications, perform surgical procedures, or provide mental health counseling.
5.2 Patient Consent
Pelvic floor physical therapy may include internal vaginal and/or rectal assessment and treatment techniques. You will be provided with a thorough explanation of all procedures before they are performed, and your informed consent will be obtained prior to any internal assessment or treatment. You have the right to decline any procedure or withdraw your consent at any time without penalty or impact on your care.
5.3 Medical Clearance
Certain conditions may require medical clearance from your physician or referring provider before beginning pelvic floor physical therapy. We may request documentation of medical clearance when clinically appropriate. It is your responsibility to disclose all relevant medical conditions and to obtain any necessary clearances prior to treatment.
5.4 Referrals
If your condition falls outside our scope of practice or requires additional medical evaluation, we will refer you to an appropriate healthcare provider. We maintain professional relationships with physicians, specialists, and other healthcare providers in the Utah Valley area to ensure continuity of care for our patients.
6. Telehealth Services
Radiant Pelvic Health & Wellness may offer telehealth consultations and follow-up sessions when clinically appropriate. By participating in a telehealth session, you acknowledge and agree that:
- Telehealth is not a substitute for in-person evaluation and hands-on treatment when such treatment is clinically indicated
- You are responsible for ensuring a private, secure environment for your telehealth session
- Technical difficulties may occur and are not the responsibility of Radiant Pelvic Health & Wellness
- The same cancellation policy, payment terms, and patient responsibilities apply to telehealth sessions as to in-person appointments
7. Confidentiality & Privacy
Your privacy is important to us. All personal and health information collected during the course of your care is handled in accordance with our Privacy Policy and the Health Insurance Portability and Accountability Act (HIPAA). We will not share your protected health information without your written consent except as required by law or as necessary for treatment, payment, and healthcare operations. 8. Limitation of Liability
8.1 No Guarantees
While we are committed to providing the highest quality pelvic floor physical therapy, we cannot and do not guarantee specific outcomes or results from treatment. Individual results vary based on many factors, including the nature and severity of your condition, your adherence to the treatment plan, and your overall health.
8.2 Liability Waiver
To the fullest extent permitted by law, Radiant Pelvic Health & Wellness, Dr. Danaya Kauwe, and any associated staff or contractors shall not be held liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to the use of our services, website, or any information provided during the course of treatment.
8.3 Assumption of Risk
Physical therapy, including pelvic floor physical therapy, carries inherent risks, including but not limited to temporary soreness, discomfort, bruising, or exacerbation of symptoms. By receiving treatment, you acknowledge that you understand these risks and voluntarily assume responsibility for them. You agree to inform your therapist immediately if you experience unexpected pain or discomfort during treatment.

9.1 Website Content
All content on the Radiant Pelvic Health & Wellness website, including text, images, graphics, logos, and design elements, is the property of Radiant Pelvic Health & Wellness and is protected by copyright and intellectual property laws. The information provided on our website is for educational and informational purposes only and does not constitute medical advice, diagnosis, or treatment.
9.2 Acceptable Use
When using our website, you agree not to:
- Copy, reproduce, distribute, or modify any content without prior written permission
- Use the website for any unlawful purpose or in violation of any applicable laws or regulations
- Attempt to gain unauthorized access to any portion of the website, its servers, or any connected systems
- Transmit any viruses, malware, or other harmful code through the website
- Use automated tools, bots, or scrapers to collect data from the website without express written consent
10. Termination of Services
10.1 By Patient
You may discontinue treatment at any time by notifying us in writing or by phone. Any outstanding balances must be paid in full at the time of termination. Unused sessions in a purchased package are non-refundable unless otherwise agreed upon in writing.
10.2 By Provider
Radiant Pelvic Health & Wellness reserves the right to terminate or refuse services at our discretion, including but not limited to situations where:
- The patient's condition falls outside our scope of practice or clinical expertise
- The patient repeatedly fails to comply with the treatment plan, cancellation policy, or payment terms
- The patient engages in behavior that is disrespectful, threatening, or unsafe toward the therapist
- Continued treatment is not clinically indicated or would not be in the patient's best interest
In the event of termination by the provider, we will make reasonable efforts to provide you with referrals to other qualified healthcare professionals and ensure a smooth transition of care.
11. Dispute Resolution
11.1 Informal Resolution
We are committed to resolving any concerns or disputes in a fair and timely manner. If you have a concern about your care, billing, or any aspect of our services, please contact us directly so that we may attempt to resolve the issue informally before pursuing formal action.
11.2 Governing Law
These Terms of Service shall be governed by and construed in accordance with the laws of the State of Utah, without regard to its conflict of law provisions. Any legal action or proceeding arising under these Terms shall be brought exclusively in the courts located in Utah County, Utah.
11.3 Arbitration
Any dispute, claim, or controversy arising out of or relating to these Terms or the services provided by Radiant Pelvic Health & Wellness that cannot be resolved through informal negotiation shall be settled by binding arbitration in Utah County, Utah, in accordance with the rules of the American Arbitration Association. The decision of the arbitrator shall be final and binding on both parties.
12. Changes to Terms of Service
We reserve the right to update or modify these Terms of Service at any time. When changes are made, we will update the "Last Updated" date at the top of this page. It is your responsibility to review these Terms periodically. Continued use of our services or website after any changes constitutes your acceptance of the revised Terms.
13. Severability
If any provision of these Terms of Service is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect. The invalid or unenforceable provision shall be modified to the minimum extent necessary to make it valid and enforceable while preserving the original intent of the provision.
